At their website. It is pretty hard to find them, because they have 2 separate systems - one for applying for study and second for chosing courses.

Foreign stay plan, Acceptance letter and Research plan if Im not mistaken.

I had no clue LA was needed. I was reminded of it in the middle of my stay.

Well, thats interesting topic - it is pretty hard to find their library, because it is not near the university (about 20min walk). Its pretty small library to be honest.

I have no idea.

Ask supervisor. Mine made copies for me when I asked nicely.

Does the foreign school/student organisation arrange special events for exchange students?:
yes

If yes then what?:
They have a lot of workshops and small conferences. Everybody is welcome and they usually invited me.

How would you assess your integration with the local students (evaluation like in school from 1 to 5 with 1 as highest):

I had a few meals with polish students. I would say that I got to know most of their doctoral students and promising master students.

Everything. You can find coffe and cups here, but thats it. To be honest it is more than expected from hostel.

Day before arrival. Just use booking.com.

What are the catering options?:
I would recommend "milk bars" - cheap and tasty polish food.

Tips for purchasing cheap tickets and other recommendations:
Cheapest way is to go by bus to Ostrava Svinov and then by bus to Krakow MDA.

I was walking all the time - It is not that big city. But there are some local trams.

Tips for free-time activities:
Dont drink beers! Their brewing industry is centuries behind ours.
Dont even speak with random people on streets who are luring you inside random businesses.
Just look around, there is a lot of nice places.
